Turner Foundation Pass Card Application (Fiscal year July 1 through June 30)

A $25 Student Passcard donation includes one individual pass to all Turner Middle School and Turner High School sponsored events (excluding dances, school plays and all KSHSAA post season events).

PASS MAY ONLY BE USED BY THE NAMED TURNER FOUNDATION PASS CARD HOLDER. TURNER FOUNDATION WILL NOT BE RESPONSIBLE FOR REPLACING LOST, STOLEN OR DAMAGED CARDS.

The Turner Foundation is a non-profit organization and does not provide any goods or services in exchange for contributions; therefore, your contribution is tax deductible as allowable by law.

Charge includes a small fee to cover the cost of credit card processing. Cards will be mailed within 2 business days of purchase.

PLEASE NOTE:

-The card must be present at the gate in order for it to be honored- we cannot accept a picture of the card

-Only the person whose name appears on the card can use the card (not your Student/Child, these cards are not a family passes)

-The ticket taker will have to read the name on the card and may request proof of identification; the guest cannot just flash the card and continue into the event

-The ticket taker will honor a receipt of purchase within three days of payment in lieu of the card, but cannot accept this as proof after that time
